Meet the Kids Where They Are
Integrating Community Social Pediatric Care in Schools
This project expands New Brunswick Social Pediatrics (NBSP) by adding a school and health liaison role in three Saint John schools. It supports children facing social and health challenges, especially those impacted by poverty and trauma, by providing timely health care, early interventions and community resources.
By embedding health care in schools, the initiative strengthens community partnerships, improves health outcomes and reduces long-term costs. The pilot will be evaluated to guide the broader implementation of integrated community health care models.

Hospital to Housed
Building a Comprehensive Pathway for Unhoused Patients through Coordinated Care and Convalescent Housing
The Convalescent Care Beds Program in Saint John offers stable housing for people experiencing homelessness after hospital care. This initiative aims to reduce hospital admissions and emergency room visits while providing trauma-informed care and improving patient access.
This project will evaluate effectiveness and expand community support through a two-phase study. Its goal is to create a scalable model to enhance health care delivery and connect patients with resources for continued health maintenance after recovery.
